
HOW TO OBTAIN
AN ID CARD
Requirements for Everyone
- You must obtain Departmental
approval before receiving a new or replacement UCSF ID card

You must bring with you at least one
acceptable form of identification
when you get your UCSF ID card at a
WeID
service location.
- If
credentials are required on your
card you
must show proof of credential prior to ID card issuance.

A
fee
is charged each time you receive a new or replacement UCSF ID card.

UCSF Campus (Students, Faculty, Post-Docs,
Staff, Contractors, and Affiliates)
Faculty, Post-Docs and Staff
Please contact
your Manager/Supervisor/PI to request a new or replacement UCSF ID card.
Individuals may not authorize themselves for a new or replacement UCSF ID card.
The authorization must come from the individual's Manager/Supervisor/PI. If you
are a new employee, ask your Manager/Supervisor/PI when you should proceed to a
WeID
service location to obtain your
new/replacement UCSF ID card.
Students
Your
School may have arranged for new students to obtain UCSF ID cards at
specific times. You should be informed of those specific times during
orientation. Please see your School's designated contact for more
information regarding new ID card issuance procedures.
- If you need a replacement card, contact your
School Administration in order to get
approval
for a replacement card,
whether or
not the school is paying for the card.
- If
the
School is paying for the card, they need to enter the dpa/fund into
the ICRD screen (they will know what this means).
- If you
are paying for the card, you must bring
written
approval from your School administration authorizing
a replacement card, with an appropriate signature. After you obtain written
approval, you need to go to a
WeID
service location with one
acceptable form of identification and the
fee (currently $35.00) in order to obtain the replacement card.

Contractors and Affiliates
If you are
a contractor/vendor/volunteer or someone who is not paid through UCSF payroll, you will need to supply the minimum
required information
to the UCSF department for which you have an affiliation. They will inform
you when you can obtain your UCSF ID card.
