PROFESSIONAL LICENSING
LICENSE IN CALIFORNIA
Any applicant requesting a
fingerprint for a professional license in California must bring a Request for LiveScan Service Applicant Submission Form. The form is provided by the
licensing agency and must be completed with the appropriate California professional board code.
The DOJ and FBI responses will be sent directly to the licensing agency.
LICENSE OUTSIDE OF CALIFORNIA
The applicant must bring the
required documentation and fingerprint cards for each state. Once the
fingerprints are taken, the applicant must follow the directions from the
requesting licensing agency for submission. The UCSF Police Department cannot
submit out-of-sate licensing request to the DOJ or the FBI.
