FINGERPRINT APPOINTMENTS
Step 1: For both UCSF campus and UCSF Medical
Center employees, your
manager or supervisor must complete and sign a
Request for Sensitive
Employee Fingerprinting Form (campus) or a UCSF Medical Center Request
for Fingerprint Form (medical center employees).
For professional licensing in
California
you must obtain a Request for LiveScan Service Applicant Submission Form from
the licensing agency. For out-of-state licensing
you must obtain the appropriate fingerprint cards from the
licensing agency.
Step 2: Either you, your manager, or supervisor, must make an
appointment at one of the UCSF Police Department LiveScan service
locations. To make an appointment, call 476-2088.
Step 3: You must bring both the appropriate completed and
signed request form or licensing cards and a valid
government photo identification to the service location.
The following are approved forms of government
photo identification:
* US
Issued Driver's License
* US
or Foreign Passport
* US
State ID Card
* US Work
Authorization Card
* US Green Card
* US Military
Identification Card
Note: A UCSF
ID Card is not sufficient identification for Fingerprinting.
